Registration and Assessment

Students' responsibilities:  

 

Students who have NEVER registered in the SEN Support Service in their current degree

(Registered student returning for a second degree at HKU should register again as academic needs may differ.)

Current registrants of SEN Support Service

Part 1: Registration and Supporting Documents (Please refer to the Guidelines on Documentary Evidence for the requirement)

☑ Completed the electronic application form

☑ Uploaded ALL the required supporting documents

☑ Confirm the validity of the recommended accommodations and whether any renewal is needed. If so, proceed with the following steps.

☑ Provide updated documentations if there are any significant changes in your disability conditions / required accommodations

Part 2: Needs Assessment with SEN Support

Make an appointment with your Case Manager and participate in the Needs Assessment and/or Needs Review Meeting(s)
(For new registrants, a processing time of at least 7 working days is required for reviewing your registration. You will then receive a notification email on how to make an appointment online.)

☑ Actively communicate with your SEN Case Manager on any changes to your needs and/or arising difficulties

Deadline for completing Part 1 AND 2 for arranging accommodations for 2026-27 Semester 1

Notes

1. If you fail to complete the above by the deadline, some support services, such as special examination arrangements, will not be arranged in that same semester due to the processing time required. Accommodations for the next semester will be arranged as normal. Students with SEN are strongly encouraged to register as soon as possible.

2. Please see the Specialist's Report Form template for your reference. This form is intended to be an easy-to-follow template form for specialists to fill in, especially for students that are recently diagnosed / needs to have an updated report. It is not compulsory. If you have medical documents that fulfilled the requirement set out in the Guidelines on Documentary Evidence (see above), you do not need to submit this form.

3. If you would like to apply for special timetabling arrangement for your examinations, you must complete the registration procedure and raise your concerns to your case manager by as soon as possible. A deadline is yet to be annouced though it will usually be within the first week of the start of the term. Please note that such request has to be supported by documentary evidence.

 

Registration procedures


01. Contact Student Support and Inclusion (Diversity and Inclusion), CEDARS

02. You will receive an individual link of the registration form through email

03. Complete the following steps on a ONE-TIME basis with the individual link:

  1. Fill in the information required on the registration form; and
  2. Upload a copy of your supporting document(s)*


IMPORTANT: The above steps MUST be completed by the deadline for that semester.


04. You will receive a notification email after submitting the online registration

  • Registration will only be successful if you have uploaded supporting document(s)

05. Student will receive a link to the booking system once they are assigned to the Case Manager. Student will need to make the appointment with their Case Manager afterwards.

*Upload only one document in PDF/ ZIP format. If you have more than one document, please upload them in one ZIP file. Max size: 15 MB.


 

Specification for documentary evidence

For documentary evidence like medical letters, professional reports or proof for past accommodations, please observe the requirements set out in the Guidelines of Documentary Evidence.

 

Privacy protection 

Registration for SEN Support is voluntary, and students are free to withdraw at any time. We are committed to meeting the University’s policies of personal data privacy protection. We ensure that the collection and release of student information are only for the purposes of service arrangement.
